Requirements:

If your child will be in Pre-K, Kindergarten, or 6th grade they will need a Health Exam. Please have this done as soon as possible and fax, bring in a paper copy, upload to TADS or email to our school nurse at mdastych@kingswoodacademy.org.

• If you have a religious objection to immunizations for your child, you may submit a religious exemption form along with the Health Exam. If you have any questions please contact the office.

Dental exams are required for Kindergarten, 2nd grade, and 6th grade.

Eye exams are required for Kindergarten and any student attending an Illinois school for the first time.

Sports physicals are required for any student participating in sports prior to the first practice. Your student’s annual Health Exam can be used for a sports physical.
