Medical Forms

Requirements

Students entering Pre-K, Kindergarten, 6th grade, and all new students will need a Health Exam. Please have this done as soon as possible. If you have a religious objection or questions about health forms, please contact the school nurse before submitting any documentation. Otherwise, bring in a physical paper copy OR upload to TADS OR email all pertinent documents to: mdastych@kingswoodacademy.org.
